Job Description:
The Lifestyle Coordinator researches, plans, organizes,
and directs a program of activities that gives the
opportunity for self-development, personal enjoyment and
satisfaction for all residents. As a member of the
Lifestyle Services team, this individual collaborates
with all community team members in providing outstanding
and diverse activities for the health center. While
having primary responsibility for a particular group of
residents, each Lifestyle Coordinator is expected to be
equally as comfortable providing services throughout the
campus.

Job Requirements:

  • Researches, develops, plans and conducts a variety of
activities and programs to carry out the objectives of
the facility by meeting the intellectual, social,
spiritual, physical, emotional, vocational needs of the
residents, while encouraging resident choice and active
participation in all aspects.
  • Will lead programs in all dimensions of wellness
classes for all levels of the community.
  • Render service to the residents such as reading and
writing letters and doing errands whenever possible.
  • Encourage hobbies and develop programs for residents
on a one-on-one basis when appropriate, wherever they
call home.
  • Fosters a working, cooperative relationship with the
families of residents.
  • Maintains network of community resources to assist in
programming across the community.
  • Encourages resident participation in outdoor events,
social gatherings and spiritual activities.
  • Participate in Care Conferences by developing an
individualized approach for each resident and
documenting special interest, attendance and individual
progress.
  • Participate in Quality Assurance program by submitting
a report each quarter on the identified goal.
  • Utilize resources within the community and facility.
Purchase and inventory craft items, equipment and
miscellaneous supplies needed for planned activities..
  • When working in clinical areas, notifies Community
Health Advocate (DON) or Household Care Coordinator
(RN/LPN) of any noted changes in resident condition.
  • Supervises residents during activities so that safety
regulations and policies are followed.
  • Completes required documentation including MDS and
other clinical components where applicable
  • Supervises volunteers assigned to Lifestyle Services.
  • Assists in the development and update of department
policies and procedures.
  • Fosters communication and serves as mentor to the
household team members in delivering on resident
preferred lifestyle activities.
  • Coordinates and maintains systems of documentation to
support programming in the six dimensions of wellness.
  • Collaborates with residents, volunteers, and other
team members on on monthly educational programs and
resident publications/communication systems including
community newsletter
  • Attends educational seminars as required.
  • Performs other duties as assigned by Director of
Campus Life and Executive Director.
KNOWLEDGE, SKILLS, AND ABILITIES
  • Prefer four year college degree in Recreation Therapy,
Arts, Music or related field.
  • Activity and Long-term Care experience preferred.
  • Prefer Certified Nursing Assistant or ability to
obtain.
  • Chauffer license or ability to obtain.
